The cost is $45.00 per month. Please pay two months in advance
(October & November + 10.00 for materials = $100).

Recorder, Clarinet and Flute will be during school hours from 1:30 p.m. – 3:00 p.m. Classes are 30 minutes per session. Violin, Guitar and Trumpet will start at 3:00 p.m. – 5:00 p.m. Classes are 40 minutes per session.
My Child needs to rent a Trumpet from the school Yes____. The school only has 10 Trumpets. It will be on a first come first serve basis.
The school does not rent Violins. Parents will have to purchase their own for their children. The minimum cost is $ 150.00. If it is your desire Mr. Veras, the Violin teacher, will be glad to help you if you send him the money before classes begin.
All Students need to bring $ 10.00 for materials for the year.

I agree to return this Trumpet - (instrument) Serial #_________________ in good condition normal wear excepted to the music teacher at the end of the rental period which is the school year. I will take full responsibility for the cost of its replacement and repair if the instrument is lost, damage of a deliberate nature, and / or damage resulting from any attempt of repairs by an unauthorized person. The School is not responsible for negligence, instruments left in any vehicle. Any attempt to return an instrument with a serial number other than that of the original instrument negates the coverage of the original instrument. I agree to advise the School of phone number and / or address changes. This contract is not transferable. Parent(s) will receive a copy of this agreement. (CC)
